چکیده

Methods Coronary artery bypass grafting (CABG) and surgical correction of ischemic mitral valve were performed in 52 patients with coronary artery disease (CAD) and IMR and 52 patients with CAD and IMR underwent only CABG. In both groups they had moderate or severe mitral insufficiency. Serial transthoracic and transesophageal echocardiograms were performed before surgery, after 6 months and after 12 months to assess grade of MR, left ventricular and left atrial remodeling, left ventricle ejection fraction. Clinical follow up NYHA functional classification, survival, events were assessed after 1-year follow up. Statistical test used were Mann Whitney U test, student’s t-test, c2 test.

Surgical treatment for functional mitral regurgitation secondary to dilated cardiomyopathy: Current options and future trends

There is an increasing number of patients with mitral regurgitation secondary to dilated cardiomyopathy. Recent data suggest that mitral regurgitation (MR) can be surgically corrected in heart failure with symptomatic improvements and favourable reverse left ventricular remodeling. However, several questions remain to be answered, regarding the optimal management of functional mitral regurgitat...

اثر PercutaneousCoronary Intervention Elective بر بهبودی نارسایی ایسکمیک میترال

Introdution: Ischemic mitral regurgitation (IMR) remains one of the most complex and unresolved aspects of ischemic heart disease that the impact of percutaneous coronary intervention (PCI) on improvement of intensity of ischemic mitral regurgitation is not well clarified. Patients with coronary artery diseases and ischemic mitral regurgitation have a worse prognosis than the patients with coro...

Effect of isolated cabg on mild-to-moderate ischemic mitral regurgitation and cardiac remodeling

Results At follow-up 56 % of all survivors showed improvement in IMR grade, 32 % presented with the unchanged valve function and in only 12 % IMR grade was more severe than preoperative. Preoperative effective ejection fraction increased from 26 ± 8% to 32 ± 14% (p = 0.002) at the follow-up. Left ventricular end-systolic volume index (LVESVI) decreased from 35 ± 16 ml/m2 to 30 ± 14 ml/m2 (p = 0...

Should moderate or greater mitral regurgitation be repaired in all patients with LVEF <30%? Mitral valve repair in patients with advanced heart failure and severe functional mitral insufficiency reverses left ventricular remodeling and improves symptoms.

Mitral regurgitation (MR) is commonly observed in patients with heart failure and associated with a poor prognosis.1–3 Mitral valve repair or replacement to restore valve competency is a well-established procedure when there are symptoms of heart failure and the primary disease is of the valve leaflets. However, recent interest has focused on functional or secondary mitral insufficiency in whic...

Downsizing of the mitral valve and coronary revascularization in severe ischemic mitral regurgitation results in reverse left ventricular and left atrial remodeling.

OBJECTIVE Data of combined mitral downsizing by restrictive prosthetic ring annuloplasty and coronary artery bypass grafting (CABG) in patients with ischemic cardiomyopathy and moderately severe to severe mitral regurgitation (MR) are rare, and little is known about the effect on reverse left ventricular (LV) and left atrial (LA) remodeling. METHODS Thirty-eight patients (70.6+/-8.3 years) wi...
